Some quick additional notes:

Section 6.5.2.1 took roughly its current form in ARIN policy 2011-3.
There have been some smaller revisions since.

The Allocation/Assignment mismatch was present in that original
policy, as was the error in the formula.


With some effort (it's not smooth) you can search revisions to the
NRPM at https://bitbucket.org/arin-rir/number-resource-policy-manual/src/master/

can help you zero in on exactly where particular changes were made,
but the interface is really rough. The following command line works
somewhat better though still not great:
