In what reality is this true?

Regional ISPs aggregate.  Content provider ISPs/NSPs aggregate.
Come to think of it, every network I can think of aggregates and
sends traffic to a higher speed uplink / trunk.

The cell phone network and ad-hoc mesh networking supported by
laptops like the OLPC, are the only examples that come to mind that
support the "meshy" global Internet.
