I hope this doesn't come as a surprise, but IPv4 addresses are NOT going to be available "at a reasonable cost" regardless of any "demonstrated need" policy. It would seem apparent that there will remain sufficient demand for IPv4 to drive market-based pricing among the folks who can easily demonstrate need and I suspect those folks are going to have way more money than "small operators".
The only deck chairs we're rearranging right now are the ones associated with who gets to collect the money. From the perspective of the small operators, I don't suspect it matters if the address space is unavailable because Speculators-R-Us has acquired it for speculation instead of Globespanning-Zillion-Dollar-ISP for renting to their customers at $20 per /32 per month (or whatever).

I believe one of the problems here has been that the RIRs have distorted the market, hiding the actual long term true cost of IPv4 thereby making the deployment of IPv6 look more expensive by comparison. However, as a proctologist will tell you, "this too shall pass, although it's going to be painful"...
