This dichotomy was never valid technical policy.  To understand it,
"follow the money."

There is a substantial difference in the fees between the groups today,
so the group that pays lower fees (i.e. end users) will resist any
attempt to merge them with the group that pays higher fees (i.e. ISPs/LIRs).
