The latter is what works for us!

I frankly think the ITU needs to stay out of this.  I would not
take kindly to any initiatives from the ITU to interfere with ICANN's 
operations in the existing RIR governance.  I likely would be 
null-routing any IP addressing that the ITU assigns out, at least for
a while.

While the ITU may be hell-bent on becoming an RIR they aren't
doing it to help the Internet, they are only interested in doing
it because they want to stick their noses into something they never
understood in the first place.
