Just to confirm current fees:

$1250 is the current initial and annual fee for IPv6 micro-allocations 
(/48) for non-members.
If you're a member, including a non-subscriber member ($500/year), 
the fee is waived through December 2006.

There is currently no separate fee schedule for IPv6 end user
assigments, as there is in IPv4.
