I am not securing systems with obscurity.  However, there is no reason to provide an easy directory/mapping of interesting private exchanges to specific subnets.

No, they are not necessarily in DNS.

I'm not a HIPAA expert, but I'm fairly sure that "security in
obscurity" is not a HIPAA-compliant standard of locking things down.
